(November 2024) COMBINE Essential Safety Net Providers (ESNP = "esnip"s) are building referral lists for organizations that provide SDOH (social determinants of health) services. 

Cor CCHA complex cases and care coordination: 
"Providers are encouraged to consider referring members for CCHA’s care coordination services if they:

Are having difficulty navigating their care or have high levels of need for assistance

  • Face complex health challenges or barriers to receiving needed care
  • Need connection to health-related social needs or non-medical resources like transportation, childcare, food assistance, or housing
  • Require additional disease or medication education for better management of their conditions
  • Miss appointments due to transportation or other challenges
  • Have chronic or complex conditions and need help with care coordination
  • Visit the ER or hospital frequently or use behavioral health or crisis services often

(May 2024)  The state's Behavioral Health Administration (BHA) and Dept of Health Care Policy and Finance (HCPF) both have roles to play in supporting the "safety net providers" programs, of which there are two: Essential Safety Net Provider (ESP), and Comprehensive Safety Net Provider (CSP). 

CSPs are essentially the large "Centers" (usually designated as Community Mental Health Centers) and they are responsible for a range of services. ESPs are responsible for one or more services, and COMBINE clinics are likely to meet the threshold for designation as ESPs.
